Job Summary:
 As clinical expert, liaison and consultant for orthopedic caregivers and families, this position is responsible for planning, coordination, integration, education and the ongoing evaluation of the delivery of care to the specialty, orthopedic population in need of Total Joint Replacement. The coordinator is responsible for assuring the delivery of integrated, orthopedic care in the pre-operative, peri-operative and post- operative settings. The Total Joint Coordinator drives care coordination by actively providing consultative screening and evaluation of patients to direct and enhance access to our services and improve patient, family, and referring physician satisfaction.
General Responsibilities:
  •        Responsible for the overall administration and operations of the total joint program under the direction of the Administrator. 
  •        Conducts the pre-admission education classes for patients scheduled for joint replacement procedures.
  •         Coordinates with care team of patient during the acute hospitalization to enhance patient satisfaction and ensure quality care including rounding during hospitalization and post discharge follow up phone calls.
  •        Collaborates with all aspects of the healthcare team through team conferences and one-on-one meetings to ensure a quality of care delivery process for patients for optimal patient care.
  •         Accountable for maintaining patient education materials to educate patients and families on joint replacement procedures.
  •        Participates in the development, implementation and evaluation of departmental and organizational mission and goals.
  •        Maintains a flexible schedule as necessary to meet the needs of patients, staff and system.
  •         Serves as liaison and subject matter resource to physicians, staff, patients, families, and professional organizations, societies, etc. to ensure that optimal patient care is provided to the orthopedic surgical patient.
  • Ensures compliance with core measure initiatives appropriate to service line.
  •        Master Degree in Nursing or related field.
  •        Current Florida RN license.
  •        Must have at least three years of related clinical experience: RN, PT, ARNP, PA, Case Management.
  •        Current BLS card from American Heart Association required.
  •        Orthopedic experience preferred.
  •        Must Pass L2 Background Check
